PRE-EMPLOYMENT DRUG & ALCOHOL TESTING
NOTIFICATION AND CONSENT
I understand that, as required by the Federal Motor Carrier Safety Regulations 49 CFR Part 382 and company policy, all prospective drivers must submit to a controlled substances test involving collection of a urine sample that will be tested for the following controlled substances: marijuana, cocaine, opiates, amphetamines and phencyclidine (PCP). I also understand that I will be placed on the employee alcohol random testing program and will be required to submit to an alcohol test should my name be selected. This test will be completed while the employee is currently working on the job.
I understand that, if I test positive for use of controlled substances or alcohol, I am not medically qualified to operate a commercial motor vehicle. I also understand I will be given a reasonable opportunity to confer with the company’s medical review officer before any positive drug test result is reported to the company.
The results of the drug tests will be maintained by the safety review officer of the company, who will report to the company whether the test results were negative or positive. The results of any tests will not be released to any additional parties, except as provided in section 40.37, without my written authorization.
POST-ACCIDENT DRUG & ALCOHOL TESTING POLICY
NOTIFICATION AND CONSENT
I understand that Wooden It Be Nice, Ltd is committed to a safe, productive, and drug‐free environment and promoting the general health and well‐being of all employees. However, this commitment is jeopardized when employees illegally use drugs or alcohol on the job, come to work under the influence, or manufacture, possess, distribute or sell drugs in the workplace. Therefore, in order to achieve the Company’s objectives of safety, productivity, health, and well‐being in the work place, and in accordance with applicable OSHA regulations pursuant to 29 CFR 1926 the Company has established the following policy with regards to post-accident drug and alcohol testing.
Employees involved in any incident which results in damage to or loss of any company property, project owner property, general contractor property and/or any other property, or a claim of work‐related injury or illness requiring medical care (other than first aid), must submit to testing before leaving either the job site or the clinic where treatment for the injury occurred. Refusal or avoidance of testing can result in severe disciplinary measures including termination. All employees and supervisors must advise the office of any incidents as soon as possible.
